The Program
Medication Review support at Pharmasave Remembrance
Patients in Brampton may be eligible for the Medication Review program with a valid Ontario Health Insurance Plan (OHIP) card when one of the following applies:
- You take three or more prescription medications for a chronic condition.
- You have been diagnosed with Type 1 or Type 2 diabetes.
- You live in a licensed long-term care home.
If the need arises, patients may also be eligible for one or more follow-up appointments throughout the year with the pharmacy team.
